If you reason for calling is a medical reason, we must know Doctors name and phone number so we can verifiy the medical issue. You may have to call prior to give them permission to release this information to us!
If you are calling because of a death in the family, we will need the doctors name, hospital, hospice or funeral home and their phone numbers. Having the arrangements at this time is helpful if requesting presence.
If you are sending a message of a new arrival, again we will have to verify the birth.
If you have not heard from the serviceperson in quite a while and it is unussual you have not been contacted, we can send a Health & Welfare requesting the serviceperson to give you a call or write you a letter.
